Please click on the link below to take you to the child information sheet form. Please fill out one form for each child that you plan to have in our care so that we have a better understanding of how we can best care for your child.

Childcare Information Sheet
Please fill out one form for each child you plan on placing in our care. You will need to print out a copy and sign this form physically (electronic signatures not accepted). Remember, if information about your child is not written down, we may not know about special needs of your child such as allergies, medical needs, etc.

Childcare Agreement
A childcare agreement form will be filled out by Itsy-Bitsy Tots Childcare for you to sign with the terms and conditions of your childcare agreement. Only copies signed by both parties are valid agreements. This form will be provided to you by Itsy-Bitsy Tots Childcare upon enrollment.
